I am in the process of collecting my documents to submit my application to your university, but I am unsure exactly what is meant by “6. Agreement for Verification of academic record”. Please could you tell me precisely what is required for this, as I do want to make a successful application to your undergraduate program.

We will officialy ask your high school about your transciprt and diploma if you get admission. Form 6 means that you are agree with it for UNIST admission.
